Why does a period cause fatigue?

Several things happen at once around the time of a period that can leave your daughter feeling wiped out:

  • Hormone shifts: progesterone drops sharply before a period, which can affect mood, sleep quality, and energy levels
  • Disrupted sleep: cramping, discomfort, and restlessness during the night can mean she is not getting the amount of sleep a teen needs
  • Blood loss: losing blood uses the body's resources, and if she has heavy periods, this can compound the feeling of tiredness
  • Iron deficiency: if blood loss is significant over many months, iron stores can become depleted, making fatigue more severe

What is the difference between normal tiredness and iron deficiency?

Normal period tiredness tends to be most pronounced in the first two or three days of bleeding and improves as the period ends. 

Iron deficiency anaemia (a condition where there are not enough red blood cells to carry oxygen around the body) causes a more persistent, deeper tiredness that does not lift after the period ends.

Other signs of iron deficiency anaemia include:

  • Dizziness or lightheadedness
  • Headaches
  • Pale skin
  • Difficulty concentrating
  • Weakness or feeling breathless on exertion

If your daughter has heavy periods and persistent fatigue, a simple blood test can check her iron levels.

Is period fatigue more common in teens?

Adolescent girls are particularly vulnerable to iron deficiency because they are growing rapidly and losing blood monthly, often before their diet has caught up with their nutritional needs.

Research in PubMed has confirmed a high prevalence of iron deficiency in teenage girls, particularly in those with heavy periods.

Even without anaemia, iron deficiency alone can cause fatigue, headaches, and reduced concentration.

What can help with period fatigue?

Practical steps that can make a difference include:

  • Prioritising sleep, particularly in the days leading up to and during her period
  • Taking vitamin C alongside iron-rich foods, as this helps with absorption
  • Exploring the best foods to eat on a period, such as iron-rich foods like red meat, lentils, beans, dark green vegetables, eggs, and fortified cereals
  • Staying well hydrated
  • Gentle movement such as walking, which can improve circulation and ease fatigue
  • Avoiding caffeine in the afternoon if it is disrupting her sleep

If a doctor identifies iron deficiency, iron supplements taken for around six months are usually recommended alongside dietary changes.

Should I take my daughter to a doctor about period fatigue?

If fatigue is mild and tied to her period, monitoring and dietary support is reasonable as a first step. 

If tiredness is severe, persistent beyond the period, or affecting school and daily life, a doctor's assessment including a blood test to check for anaemia is a good idea.

Can iron supplements help if her periods are heavy?

Yes, iron supplements can help with fatigue caused by heavy bleeding, though they will not make the periods themselves lighter.

How long does period fatigue last?
