You could quietly ask again and say, “It’s an emergency because I’m on my period, and I really need the toilet.”
If saying it out loud feels embarrassing, you could write it in a notebook or on a note and show the teacher privately.
If they still won’t let you go, speak after class to a trusted adult at school, like the school nurse, head of year or someone on the pastoral team.
You could also tell a parent or trusted adult, who could contact the school for you.
